ODONTOGENIC CYSTS

Odontogenic cysts arise from tissues involved in tooth development.

Broad groupCystKey association / origin
Inflammatory odontogenic cystsRadicular cystNon-vital tooth; most common inflammatory cyst
Residual cystRemains after extraction of causative tooth
Paradental cystVital tooth; inflammatory; usually mandibular molar
Buccal bifurcation cystVital mandibular first/second molar; children
Developmental odontogenic cystsDentigerous cystAssociated with crown of unerupted tooth
Odontogenic keratocyst (OKC)Often posterior mandible/ramus; PTCH1; Gorlin syndrome
Lateral periodontal cystLateral root surface of vital tooth
Botryoid odontogenic cystMultilocular variant of lateral periodontal cyst
Gingival cystSoft tissue; developmental
Glandular odontogenic cystUsually anterior mandible; can be aggressive
Calcifying odontogenic cyst (Gorlin cyst)Calcifications; ghost cells
Orthokeratinized odontogenic cystOften posterior mandible; associated with impacted 3rd molar
Eruption cystSoft tissue over erupting tooth
Odontogenic cyst of infancyGingival/alveolar cysts in infants

NON-ODONTOGENIC / DEVELOPMENTAL CYSTS

CystTypical location / association
Nasopalatine duct cystIncisive canal/anterior maxilla
Nasolabial cystSoft tissue of nasolabial fold
Median palatal cystMidline palate; rare entity
Median genioglossal/mandibular cystHistorical/rare designation
Nasopharyngeal duct cystPosterior nasal/nasopharyngeal region
Epidermoid cystFloor of mouth/soft tissue
Dermoid cystFloor of mouth
Thyroglossal duct cystNeck, usually midline
Branchial cystLateral neck

PSEUDOCYSTS

These do not have a true epithelial lining, so technically they aren't true cysts.

PseudocystTypical location / feature
Simple/traumatic bone cystUsually posterior mandible; scallops between roots
Aneurysmal bone cystExpansile, blood-filled lesion; often younger patients
Stafne bone defectLingual posterior mandible below mandibular canal; static bone defect

SYNDROME ASSOCIATIONS

SyndromeClassic dental/jaw finding
Gorlin–Goltz syndrome⭐ Multiple odontogenic keratocysts
Gardner syndrome⭐ Multiple osteomas + impacted/supernumerary teeth
Cleidocranial dysplasia⭐ Multiple supernumerary + unerupted teeth
Noonan syndromeMultiple odontogenic abnormalities can occur
Marfan syndromeDental/skeletal abnormalities; not a classic cyst syndrome
Ehlers-Danlos syndromesPeriodontal/connective tissue manifestations
Neurofibromatosis type 1Jaw enlargement, osseous changes, impacted/displaced teeth
Cherubism⭐ Bilateral multilocular giant-cell lesions of jaws
Hyperparathyroidism⭐ Brown tumors; can mimic giant-cell lesions
Multiple endocrine neoplasia / related syndromesCan have jaw lesions depending on syndrome
Peutz-JeghersMucocutaneous pigmentation + GI polyps; not a classic jaw cyst syndrome
